In the Philippines, the stigma surrounding abortion is another factor that makes it difficult for a woman to seek treatment after an abortion. Some women report feeling ashamed and intimidated by health workers, and in some cases women do not receive painkillers and anesthesia, or treatment is delayed or denied altogether.4,5 Others report that they are threatened with passing the baton to the police. Doctors themselves report that they have a prejudice against patients after abortion, with some believing that these women have committed criminal crimes. Other health care providers may struggle to properly manage complications when women hide the cause of their medical emergency. While some doctors secretly perform abortions in clinics, the fees of 2,000 to 5,000 pesos ($37 to $93) were too high for many Filipinos in 2005, so they instead buy abortions on the black market, such as from vendors near churches, sari-sari shops and bakeries. [4] In 2006 [Update], two-thirds of Filipino women who had an abortion tried to induce themselves or seek solutions from those who practice folk remedies. [5] In 2011 [Update], hundreds of thousands of people ended up in hospital each year due to unsafe abortions, according to the Ministry of Health,[4] and 12% of all maternal deaths in 1994 were due to unsafe abortions. Some hospitals have refused to treat complications of unsafe abortion or operate without anesthesia to punish patients.

[4] In 2000, the Ministry of Health expanded a 1998 directive entitled Prevention and Management of Abortion and its Complications (ACPM), which aimed to treat complications of unsafe abortion in order to improve the availability of the quality of humane post-abortion care services through competent service providers, compassionate, objective and non-judgmental in well-equipped institutions, which are complemented by a supportive environment. [6] [7] In 2018, a new government policy that strengthens the national framework for post-abortion care clarified the legal and ethical obligations of health care providers and provided formal ways to redress abuse. [8] • Unsafe abortion carries significant risks for Filipino women: about 1,000 people die each year from abortion complications, contributing to the country`s high maternal mortality rate. Tens of thousands of women are hospitalized each year for complications from unsafe abortion. • Policymakers and government agencies should educate the public about contraception, ensure adequate funding for contraceptives, and remove barriers to the acquisition of methods, especially among disadvantaged populations. To support the destigmatization of follow-up, the government should train more providers to use safer and less invasive methods of care. In Ireland, where more than 75% of the population identifies as Catholic, the abortion ban was lifted by referendum in 2018. On May 24, the country voted overwhelmingly to liberalize its divorce laws by removing a clause that previously limited divorce to spouses who had been separated for four of the past five years. These changes follow a steady erosion of the political power of the Catholic Church in Ireland, which has been heavily influenced by decades of child sexual abuse scandals. In the Philippines, clergy abuses have remained largely secret, as Church policies give bishops the ability to delay investigations and priests are rarely charged in criminal courts. The Church, which Filipinos trust more than any other institution, remains immensely powerful. Parents are generally strongly opposed to abortion.

In the Philippines, some methods and providers have a higher risk of serious complications: seventy percent of women who have used massage or catheter insertion have experienced a serious complication, compared to 13% of those who have received D&C or MVA.4 As mentioned earlier, larger proportions of poor and rural women use unskilled providers or riskier measures than non-poor women and Urban. and therefore they experience disproportionately more serious complications. Finally, if follow-up care is delayed, inadequate or not administered at all, mild complications can become more serious or acute, eventually affecting long-term health and well-being.
